This might or might not be relevant.

Our USB serial driver would sometimes drop the first packet after
we did a usb_unlink_urb on a bulk in endpoint.  We never fully
determined what was happening; we no longer unlink the read urb
and the problem has gone away.

Are you using usb_unlink_urb on the bulk in endpoint?
