Just a thought, spamassassin can be invoked in two ways - as spamassassin
or as a server client thing as spamc. If kmail is invoking spamc then
spamd needs to be running - you need to start it like any othe daemon.

spamd/spamc is far more resource frindly.

Also found this, dunno if it is relevant as you haven't told us which
version of kde/kmail u r running.
