Umbrello (http://uml.sourceforge.net/index.php) is pretty good if you
know how to work with persistent layers and classes in UML. The SQL code
generated is compatible with MySQL

Other option can be a combination of DIA
(http://www.gnome.org/projects/dia/) and Dia2SQLpy
(http://freshmeat.net/projects/dia2sqlpy/).

Depending on your distro DIA and Umbrello can be installed using
yum(ex).

fresmeat.net and/or sourceforge.net may offer other combinations but
didn't try them.

I use Umbrello.
