This is what I use on the system my mirror runs on:

This is a RAID 1 mirror on a pair of 18Gb SCSI drives running RH 9. Each mountpoint is striped, except for / which is mirrored. You really need an /etc/raidtab as well as /etc/fstab to correspond with this so it all makes sense.
