As you probably realize, based on what you wrote (above), PDFs can be either 
scans or conversions.  My recollection is that even if conversions, getting 
the structure isn't going to be easy.  Have you looked into whether any 
available scanner software can read a pdf directly?  If so, this might be the 
best way to move forward.
