If gesture code is hardcoded to use 4/5 (or to use e.g. relup/reldn) then that would actually explain the bug perfectly. Like Peter said, `xinput set-button-map` should work around the issue for now, but I'll try to see if I can get my hands on a bamboo and find where/how gestures are handled.
