either negative text indent or absolute positioning off screen is the best
way to go - if you need to have content heard but not seen.

sounds like an ugly hack to me but maybe there isn't another way right now?

Do any screen readers support some kind of metadata or semantic markup that could be used to embed such content in the page?

Microformats could be good too ..
(though they aren't designed for hidden data they could potentially be used to help find and describe important visible data on screen readers in the future)
