I have tried the old-trunk version and still have the same problem.
 
The plone.app.form egg has been installed and can be located here: 'E:\Zope-2.9.3\bin\Lib\site-packages\plone.app.form-0.1dev-py2.4.egg'   and in  'my zopeinstance/plone.app.form/plone/app/form' directory there is a pageform.pt file?  Is this how it should all be located?

Glancing at it, it looks like you are missing hte pageform.pt file. I
checked my install, and I do have it. Try installing the "old-trunk"
branch of plone.app.form:
https://svn.plone.org/svn/plone/plone.app.form/branches/old-trunk
