I have tried the old-trunk version and still have the same problem.
The plone.app.form egg has been installed and can be located here: 'E:\Zope-2.9.3\bin\Lib\site-packages\plone.app.form-0.1dev-py2.4.egg' and in 'my zopeinstance/plone.app.form/plone/app/form' directory there is a pageform.pt file? Is this how it should all be located?
