David,
Maybe you've installed the old-trunk version of plone.app.form without
removing the new "trunk" of plone.app.form?  What I recommend is
following these instructions again from scratch.  I have just updated
the listen-bundle to pull "old-trunk" correctly.  I've also just run
these instructions successfully.  Let us know how it goes:
