Woohoo - setting the spoolfile has fixed it. Now when I fire up mutt immediately logs me in to the remote server (using name rather than address works fine) and fetches mail from INBOX. Using 'c' lets me select subfolders as before. I still don't have mailboxes set, but as far as I can see everything is now as expected.
