We had that problem and we found out it can happen due to the following two 
reasons:

1.       Corrupt file

2.       Too many people defined explicitly in the security settings (limit is 
3000 if I remember correctly)

We have a MOSS Site that has a broken "Manage Content and Structure" link.  If 
I turn the stack trace on I get the following message:

Other sites that are on the same server (but in a different hierarchy) function 
properly, which hints at a content issue.  So, we tried restoring a test site 
from a backup, and then systematically removing content, but the link never 
works again.

A look at the SQL Server trace reveals that the last query that is attempted is 
performed twice, hinting at some sort of retry.  Running the same query from 
SQL Server Console succeeds without issue.

Anybody have any ideas?

We do have a number of sub-sites, which have sub-sites etc.  The URLs to the 
sites are getting quite long.  Could we simply be running out of space in a 
variable?
