They are not extracted from the book, but updated manually. Please see 
packages/man-pages/Makefile. It contains this:

NM= man-pages
VRS= 2.63
DIR= $(NM)-$(VRS)

FILE= $(DIR).tar.bz2
URL-$(FILE)= http://www.kernel.org/pub/linux/docs/manpages/Archive/$(FILE)
SHA-$(FILE)= 025d341ad9d197bd16108cbe80182c1c23369f58

However, this cannot give a 404 error.

This is the wrong guide for you. You want to build the CD from sources, 
but according to modified instructions. The guide is about altering the 
already-downloaded binary CD.

Your task can be done by editing the CD Makefiles.

Yes. Unfortunately, there is no such guide for 6.3. It will appear soon,

Device mapper is built later by the normal build process.
