Actually now that I think of it, I think I've seen this problem - I just
had no idea what caused it.

Sometimes when using LFO controllers to control volume (or instrument
knobs), where the LFO waveform was a type of pulse wave (just sequences
of 1's and -1's), I've sometimes noticed some skipping of the
off-cycles, like maybe the controller data didn't get updated properly.
This has happened on both export and playback.
