One notorious MS culprit is MS Project. It will only print to the default
printer and the default page size on said printer. So, if you change things
from within Project, you change all the defaults for your machine and that
printer (unless you are using NT 4 and an NT 4 print server, in which case
it comes out on the *server's* idea of the default page size, no matter what
you tell the driver at your end). This is particularly annoying when the
user sends the E-size Pert chart and then, without thinking (a common
occurrence), prints their E-mail by pressing the evil button in the Outlook
toolbar. Yes, the 3 page E-mail comes out on 3 E-size sheets.
I can imagine that MS Project is not the only one to do this...

I might suggest putting the Samba print commands into a script...
The most probable cause is Windows related:
FIBKAM - Fault Is Between Keyboard and Monitor
They are using the wrong printer. I have seen the 'mystery changing
default printer' symptom a lot. Apparently folks change their printers
in the Windows printing setup from one application and forget to
change it back.
I suspect your DNS system MAY repeat MAY (low probability)
be causing problems. But if you have a long TTL value on the
DNS entries (hours) then the problem should be persistent.
