kernel and initramfs on a usb key, floppy, cdrom, or other kind of local
storage medium.

Google for syslinux, this will allow you to do something like this.
You'll have to do some research on your own.

Any particular reason why you don't want to/can't have a dhcp server on
your network?  Did you know you could use a server with a 2 nic setup to
isolate the ltsp dhcp server from the rest of the network?  This would
be easier.
