You should first try to run LTSP on a local installation to understand how it 
works.
Using Ubuntu seems the best to try first as you can have nothing to configure
(but please read the manuals)

It is possible to specify the server IP address using an etherboot boot floppy.
You can customize the boot floppy on the site rom-o-matic.net
The boot floppy can be a CDROM or a harddisk image.

You will need a very large and good internet connection.
As an example, try to setup LTSP on a local network using a hub then using a 
switch. 
You will see a huge difference (it will not work using the hub). 
Network latency is the main problem.

In your case I would suggest to use something like NX - even VNC will certainly
work better than LTSP.
