sorry, no scanner available to me. W. Apel _notation_ p75 reproduces the woodcut, p76 has a modern typeset chart which adds another form (1-,A,F,L,Q,X) from A Schlick 1512 (Tabulaturen etlicher lobgesang und lidlein).
