Hello,
As a first step, get a version of xforms which uses libc6, and not
libc5. There are numerous problems when mixing the two.
Then it seems that lyx has not been able to find its layout
files. Could you show us what the screen output of configure was? I am
interested in the part that probes your latex configuration.
Do you have latex installed?
