It's still there, but only when the cursor is on a heading
environment. Put your cursor on the same paragraph of a title and
you'll see the entry under the Insert menu (the shortcut is Alt-A,
then 1).

Actually, yes and no. I just tried a small test document (article
class). In a section heading, the insert menu has the item. In the
article title, though, it does not (but formerly did). This is on
2.3.6.1. I suspect this is a bug.
