It's still there, but only when the cursor is on a heading
environment. Put your cursor on the same paragraph of a title and
you'll see the entry under the Insert menu (the shortcut is Alt-A,
then 1).

Sorry, my bad. It's present in the title field with the AMS article
class (which is where I usually employ it), but not with the generic
article class (where it is unsupported?).
Rich: What class are you using?
