As it is a style file (not a class file) it might work out of the box whith a
\usepackage{utathesis} in Format>Document>LaTeX-Preamble.
Of course one would need to use ERT for commands provided by utathesis.sty
Also, page-settings and the like should be on "default" for not overwriting
the settings of the style file.

I would also suggest running reLyX on the sample LaTeX files and see what
this gives. If there are too many errors, have a look at the sample text
files to see which basic class is used.
