Well, I'll list some steps but wont go into the details:

1. Have a look at the variaous letter classes that come with LaTeX/LyX
   (g-brief, dinbrief, letter, ...)
   and find out which one is the best approximation of your wishes.
   (BTW: there is a revised dinbrief layout at
    http://wiki.lyx.org/pmwiki.php/Layouts/Dinbrief
    its my favourite (for obvious reasons))
   
2. Set up your private template directory. (A template is created like
   any other lyx file, the only difference is, that you save it in the
   templates folder (which is configurable)

   See, whether there is already a template for the chosen layout. If
   so, copy it to your templates folder (otherwise just create a
   document of witht the chosen layout and save it there)
   
   Fill out all the non-changing data (backaddress, telephone, ...).
   I have e.g. two templates for private and university letters.

   
3. The big header: In dinbrief, you would use the My_Addres style for
   the header of the first page.

   Is your headed paper already printed?  -> insert a \vspace*{3cm} as
   ERT.

   Do you have the header in electronic form -> include it in the
   template.
