Completely blank or it has the name of the image file where the image should be?
I get the same behaviour with TeXshop after I convert my LyX to laTeX, so I'm guessing I have something misconfigured somewhere, but WHAT???

try going (using the QT interface) to Layout->Document->Layout (AKA Document Style) and make sure that the "Options:" line does not contain the word draft. Also does it work when you look at DVI instead of PDF? Good luck.
