Now I think I know what the cause of the problem is. If someone else confirm 
it, I will open a bug entry.
It is due to a tricky combination of mouse and keyboard behavior.

Steps to reproduce the problem:
1- Click on the inset with the mouse and do NOT move the mouse pointer (i.e. leave it on top of the inset). Then the frame appears and then cursor is placed in the inset.
2- Go out of the inset with the keyboard. Then the frame does not disappear

Sometimes the frame also remains when the mouse pointer is moved out of the inset, but I did not manage to reproduce this behavior in all cases.

These kinds of bugs are hard to reproduce sometimes. Pay attention to how you move the cursor out of the inset: Mouse or keyboard; what keys you are using, if the latter. If it's using the keyboard, where is the mouse? Over the inset? Over a different inset? Over the toolbar?
