This must work without problems. What is your LateX system, what your OS?
If you are on Windows, please update it via the Miktex update program that is linked in Windows
Start menu. Finally reconfigure LyX and try it again.
If the problem persists, delete one included file until you found the file
causing the problem.

You get this, by setting the Headings style to "headings" or "fancy" in the document settings->Page
Layout. With "fancy" you can also customize the header/footer lines. The LyX UserGuide describes
this in sec. 3.1.3 "Document Layout".
