As far as I know, the back cover image should be the very last page,
so it should come below or beneath the earlier PDF page, just like
it's already doing. So you're already doing that right.
As far as the front cover, you need to do all sorts of fancy footwork
to prevent the cover image from being bigger than the page minus
margins, thus pagefeeding. Here's how I solved that problem:
