A few likely and easy first things to verify are:

Make sure you have a good drive belt. If you or someone didn't actually buy
a new FRW8.5 belt, then it needs one.

Make sure the disk is a DD type (720k), not HD (1.44M). DD have only one
hole in one corner. HD have two holes in two corners.

Make sure the disk write-protect window is open.
