If you live in Canada, then you can get the Dual Electronics XGPS150A.  I'm not 
sure why the price is so high in Canada's Amazon store ($155 vs $100 in the 
U.S. store), even with the slight difference in the dollar conversion.

Dual Electronics posted a long list of the iPhone GPS apps that it demoed to 
work with its device.

MotionX GPS Drive also works for Canada (U.S. and Canada only). If you decide 
to get a GPS receiver, I would try this out first by buying the app for $0.99 
which includes one month of voice-assisted guidance, because you have to be 
sure that you can download the maps.  Navigon Canada is currently $49.99, but 
has been as low as $29.99 in the past.  And Navigon North America (both U.S. 
and Canada) has occasionally been on sale for less than the price of Navigon 
Canada (as low as $29.99).  These apps would have complete maps.

The main issue with MotionX GPS Drive on an iPod Touch (or any instance where 
you want to download a map), is that you have to pre-plan routes while you're 
on an internet connection if you don't have a personal WiFi device.  If you 
deviate from the route, so that you are outside of the area of the downloaded 
map, then the app will try to rely on an internet connection to get the 
additional data.

The other thing you might be able to do is get an older iPhone 3GS that is no 
longer used with a data plan.  A number of users may have upgraded their 
iPhones, and fulfilled their contracts.  In the U.S. AT&T and other carriers 
are offering existing customers an 8GB iPhone 3GS for $9.99 with 2 year 
contract.  (It's the contract that costs the money, of course!).  If you get an 
iPhone 3GS plus a pay-as-you-go data plan, you could put any GPS apps you want 
on the device, and still use VoiceOver.  That could be even more cost effective 
for you in the long run than getting a GPS Bluetooth Receiver.
