Hi!

I only have the Apple TV 2ND generation, as far as I know the 3rd generation 
hasn't been released yet but regardless of that they will both run IOS 5.1 and 
thus share msot of the same controls.

No, you don't need sighted assistance to turn Voiceover on, I forget exactly 
how it works initially now but I think from memory that the Apple TV - if the 
last update has been applied - comes up talking and you're prompted to press a 
button to turn Voiceover off if you don't need it.

Regardless of that however, its easy to turn Voiceover on if it isn't already 
enabled, you can go to the General menu, go to Accessibility and turn Voiceover 
on from there, the menu system is easy to navigate and by default you'll hear a 
"click" sound as you navigate the menu system, even boundary clicks so you know 
when you're at the first or last option etc.

Once you have Voiceover up and running then everything should be pretty 
straight forward, Apple products as a rule don't require much documentation 
hence that's why Apple sells so well but the products can at time require you 
to spend some time and have some patience working out where things are, what 
various options do and so on.

which may be of assistance, a "hands on" demo of the device is given so you 
know exactly what the device is capable of.
