I  raised this question on another list and on that list I was told about
Hearing aid mode on the iPhone  . Basically switching the iPhone into this
mode completely removed the interference playing in my hearing aids. So I
can now use it normally as a phone without having to adopt all these
strategies.

For reference Hearing aid mode can be turned on in settings general
accessibility and is in the hearing section after the vision section which
contains voiceover.

I often do that and it works, but not all the time. I've also touch ed where
the proxy sensor is and that sometimes works.
