I regularly crimp up custom length Mac modem cables. The inner 2 pins of the
RJ11 go to the outer 4 of the BT plug. I usually push a suitably cut back
cable into the BT connector, using all 4 centre pins. Then at the RJ11 end
cut out the centre 2 of the 4 wires, and bend in so they go into the middle
2 pins, then crimp. Make sure you get the correct amount of the outer
stripped back, so that the individual insulated wires reach the end of the
respective RJ11 or BT plug, and the outer sleeve is held tight when the plug
is crimped.
