Along with checking the cookies, you can just remove the entry from the 
Auto-fill list rather then unsubscribing and re subscribing. Go to 
Safari's preferences, click on the edit button for whichever one of the 
three storage places is appropriate for the site and then select that 
form from the list that will appear, delete it, then go visit the site 
filling in the form as need be and then save in Auto fill when you know 
it is okay.
