(1) Download the installer from Apple. This will show up as a package in the 
Applications folder. It's called something clever like Install OS X Mountain 
Lion. Don't install off this copy in the Applications folder because Apple 
deletes things after an install!

(2) Show the package contents by right- or control- clicking the Install OS X 
Mountain Lion file.

(3) Drill down to get to a file called InstallESD.dmg. This is the installer 
disk image. I think it's under Contents/Shared Support, but I don’t  have an 
image handy, so this is a guess.

(4) Use your favorite disk munger to make it into a DVD or a flash. You can do 
it with Disk Utility. I usually use Toast.

If you do the disk route, you'll need a double-density DVD blank because it's a 
smidgen too big to fit onto a standard 4.7GB disk after it has been expanded. 
But, I highly recommend using an 8GB flash drive because using it to install an 
OS is a lot faster off a flash. (Think hours faster.)

To use it, just use the DVD or flash as a boot drive and follow the prompts. 
I've made several of these for people, and they work fine.
