FWIW I get into similar scenarios if I change my password in ways that the 
keychain doesn't know about, like using passwd from the command line or using 
sys prefs from one account to change the password in another account. Blowing 
away the keychain is the only solution I've found (which is painful if you have 
all that iphone/ipad provisioning stuff in there...).
