Earlier today there was no Preview icon in the Dock (and I could not empty a 
PDF from the trash) and yet the Preview app showed up in What's Keeping Me? as 
the app responsible for the trash not being able to empty the PDF.  Does this 
mean that the Preview app is really still running, or that the What's Keeping 
Me? app also was not notified?

I even think that one time I looked at the Force Quit panel and saw the Preview 
app listed as running, even though it was not in the Dock.

If indeed the Preview app is not really running, then there are a LOT of failed 
notifications (Trash, Force Quit, What's Keeping Me?, others?).
