This seem to be a chronic Apple problem: "OG" accounts established back before 
there were "security questions" suddenly sprout a set of random security 
questions to which Apple cannot possibly have the correct answers in the first 
place.

The only solution I have ever found to this is to call Apple and drill them a 
new one.
