Hi!

For “ë”  press option u voiceover will then say ¨ then press a vowel and the 
Mac will convert it to a umlaut accented vowel.
This has been true for over 20 years at least for US English keyboards. I 
expect if you switch to German keyboard layout the umlaut is already a 
dedicated key.
