I managed to get the normal ones to read.
How I did this was to uncheck "use smart quotes and dashes" found in system 
preferences, keyboard, text tab and the above check box, which I unchecked.
I then opened up the Pages document and unchecked the smart quotes instructions 
you told me. Then typed some text with quotes before and after in pages and 
with the punctuation level set to most, this time, Voiceover read the quotes 
before and after. 

So would this be an accessibility issue with Voiceover not being able to read 
smart quotes but can read the normal ones. 

What's the point of smart quotes anyway. I mean, why would one use them 
compared to your normal ones. Is it a visual thing for sighted folks?
