Hi there
It sounds like that speech just got turned off somehow, still leaving voiceover 
running. I have had this happen along time ago that is, in one situation where 
the braille splay that I was running was running and the speech was not. I 
haven't had this happen in Mountain lion or Mavericks.

Another way this can happen is if you have the trackpad on and you somehow tap 
twice with three fingers on the screen. You would think that that would be hard 
to do, but I have done it. Make sure the trackpad is running and take three 
fingers and tap twice on the screen and see if that turns speech back boy. This 
is the same command as is done on the iPhone for turning speech on it off and 
leaving voiceover running at the same time. I hope this helps.
