I know it is not a volume issue, but would mute turn itself on?

I suspect there is a simple solution for this problem.
I have an ibook running Leopard.
It is largely presently for training, but I did have someone use the
laptop for an on line task.
I did command f5 to turn voice over off so the person could use the
computer, run safari swiftly.
when we finished I did command f5 again wanting to restore voice over
before I shut down the machine.
However it simply beeped at me.  We did check together, finding that
voice over is listed  as running.
I shut down waited a couple of days, then tried turning the machine on
again.  still no voice over, and again command f5 is not changing anything
but a beep.  a second check this afternoon indicates that voice over is
running.
I suspect a mute of some kind , but I am not sure how, or what steps are
needful to correct the situation.
ideas?
