How do you know it failed? As with iOS, the volumes with headphones in and out 
are independent. In other words, it may just be that you muted your Mac with 
headphones in, so now it mutes itself when you re-connect them but is not muted 
when you remove them. Verify your volume settings, or even headphones, aren't 
the problem first. I've had a couple big scares along this line, only to have 
it turn out to be nothing.
