Hello Alex,

You have to press the VO-Tab command before each time you use the 
Ctrl-Option-Cmd-t Make placeholder text command but it should work every time. 
I’ve been using this for years, and in Pages09 I created a different shortcut 
for Make placeholder text to avoid having to use the VO-Tab command.
