I just want to clarify something Chris said here which has confused a lot of my students. When we say Vo+M M, we don't mean hold the voiceover keys down: ctrl+option, while hitting the letter M, release all 3 keys, then hit the letter m all by itself. That won't do it.

This is why I never say vo+M M.  That's very confusing.

I always say vo+M, so hold down your voiceover keys, ctrl+option. While they are both down, hit the letter M. Now, keep your voiceover keys held down, and hit the letter M again, a second time.

This should focus you on menu extras, and now you can vo+left and right arrow horizontally across the extras bar. When you find the icon you need, simply press vo+space on it, and then you should be able to navigate the menu.
