I am trying to get a handle on this myself. I think what is happening is when a page is reloading or refreshing, VoiceOver’s focus goes back to it, even if you have moved away from Safari. There used to be a VO setting that indicated something about the focus going to a newly loaded webpage. This seems to be gone, but I’m wondering if this is now the expected behavior.
