So long as I interact with text, I've never had VO start reading from anywhere other than were I am in the text. I use plain arrows with Quick Nav off, and have used OS X for four years. This might be one of those things that Apple hasn't addressed because they can't reproduce it. Not to say it isn't a problem for some, but it might be fare less common than those affected imagine it to be.
