That’s interesting,

I watched Dead Pool last week on my iPad and tried it also on my iPhone,

On both devices audio description started up automatically which I assumed was 
due to voiceover being on when the movie started.

However that doesn’t work if the movie doesn’t have AD available in it.

Just a thought, maybe try it on a movie you know for a fact has AD like harry 
potter goblet of fire and on wards,

I also don't see any place to adjust this on my phone.  the only thing I'm 
seeing under settings pertains to cellular data usage and streaming quality.  
Where do you find the accessibility options in the phone app?  Both my Apple TV 
and my phone have the latest updates.

Yes to both, and in both cases, it's basically the same. Go to Settings > 
General > Accessibility, and find/enable "use audio descriptions when 
available". From there on out, iTunes/Videos will play descriptions for any 
content you have that includes them.
