Hey Chris. Actually, I've never had to be in the Skype Window for Command+Shift+R to work. There have been times where I've been in another app like Safari and Skype interrupts what I'm doing when there's a call. I can press Command+Shift+R to answer the call or command+Shift+H to decline it and in both instances it returns me to the app I was in before the call. You do however need to be in the Skype app to end the call though so you're right about that.
