The reason you've been able most likely to hit command+shit+R is because
when an active call comes in, usually it brings that window to the
foreground automatically on top of whatever window previously had focus... I
could be wrong on this, but I know for me at least, there have been times
the keystroke didn't work until I alt+tabbed over to the window... go
figure.

Hey Chris. Actually, I've never had to be in the Skype Window for
Command+Shift+R to work. There have been times where I've been in another
app like Safari and Skype interrupts what I'm doing when there's a call. I
can press Command+Shift+R to answer the call or command+Shift+H to decline
it and in both instances it returns me to the app I was in before the call.
You do however need to be in the Skype app to end the call though so you're
right about that.

If the ladder, then make sure you're focused with Skype's incoming call
window in the foreground, then, command+Shift+R to answer, and again,
provided you're in the foreground with Skype, Command+Shift+H to hang up.
