So, if you are not mixing languages in paragraphs you should be in good shape. In pages, a(at least after I enabled Hebrew), there is a menu item under format->Text to reverse the direction of the selection. Certainly not as easy as the keystrokes. I also looked at the keyboard shortcut settings area and there is no way to redefine the keystrokes to control text direction. So, a suggestion to the Accessibility folks could be to allow the control-option-command-arrow keystrokes to be remapped in the language area of keyboard shortcuts. Otherwise you are stuck with the format menu, or turning off VoiceOver changing the direction and then turning on VoiceOver. I even looked to see if I could find a way to adjust this with AppleScript.
