Makes sense. So in the case where I'm calling from iPhone 6 to iPhone 6 on Verizon, the gating factor sounds like whether we're both on LTE at that moment in time. When it works, the audio quality is startling.

You’re exactly right.  To get more specific to his questions.  Why does it work 
sometimes and not others.  Being a verizon user I can speak directly to this 
because you’re using the network I helped build.  When I talk of my time at 
verizon it was working on the very thing you’re asking about among others.  SO 
here’s the drill as Janina correctly stated there are all different codecs that 
support different qualities.  The old phone network used 8KHz or 64K channels.  
(56K of payload and 8 K of framing for old networks advanced to 64K channels 
with only 8 K of framing for every 24 combined channels and so forth.  More 
recently everything on the phone networks are moving to data networks with IP 
backbones.  This means you don’t have fixed channel sizes any more and you can 
use more data per call and we do.  As mentioned there are really clean high 
bandwidth audio specifications but the one we use tries to find a balance 
between bandwidth usage and quality.
        In verizon’s case, only LTE supports this new standard.  When you’re on 
3G with Verizon and you place a call your 3G session disconnects and you 
establish an old 1X voice only channel.  This channel supports at most about 
128K which means you have enough head room for one 3 way call initiated form 
the handset.  When you’re on LTE and Voice over LTE is enabled you can leave 
the LTE channel up and use that facility with a lot more bandwidth to carry the 
call.  This means we can have calls with 128, 384, 1024 and more K bandwidth.  
Video calling is also included in the VOLTE spec so it’s possible to send voice 
and data natively through the phone network not using FaceTime or Skype or some 
other third party software.
        Why does it work sometimes and not others?  Could be several things.  
In your case you need to both be on LTE if you’re placing a verizon to verizon 
call.  If you’re calling another network you your self need to be on LTE and 
the far end carrier needs to not only support HD calling but needs to exchange 
HD calls with Verizon.  T-Mobile does this as an example so if you called a 
T-Mobile customer and you’re on LTE you should get this quality audio.
        Does it work between brands of phones?  Yes, this is an open standard 
so Android devices can talk to iPhones and both can talk to Microsoft phones if 
the wide band calling is enabled on all accounts involved and all devices are 
on the appropriate networks for HD calling.

There are multiple codecs that can be used for telephony. Even the major
carriers have started using a higher quality codec when the equipment
supports it. You'll see this advertised as "HD Voice," by T-Mobile, for
instance.

The truth is that even "HD Voice" isn't the best available to be used,
but both ends of the call have to support the same codec (or better).
Else, down sampling and the resulting poorer quality results.

Like the adage says: garbage in, garbage out. And conversley, quality in
and maybe quality out.

built a custom video and audio conferencing system.  It replaced the one 
Verizon offers.  We actually took great care to have good quality audio along 
with fantastic video.  We supported from standard definition video up to 4K 30 
FPS non drop and on the audio side everything was G722 wide band so you’d get 
audio quality better than Skype calls in most settings.  On the phone side we 
supported G711 and G722 which means you either got something as good as a 
standard phone call or if your phone supported it like many cell phones do we 
would go wide band so again you’d get that nice crisp full audio.
        The users liked it so much they scrapped the audio only conferencing 
service and before we knew it we had 5+ million minutes of business per week.  
We’d support thousands of parallel calls and were able to maintain crisp audio
